How To Get Started At AVCTC
We offer a complimentary phone screening to learn more about your child to explain our process, and to answer any questions you may have. The purpose of this call is to ensure we are a good fit for you and your child’s needs
TWO Obtain Referral
Once we determine that you’d like to schedule an evaluation, you will need to obtain a signed prescription form from your doctor that includes a diagnostic code. This will allow you to try to obtain reimbursement from your insurance company.
Contact our office to schedule your evaluation or to get on our wait list. In order to get on our waitlist or schedule an evaluation, we require a $100 non-refundable deposit
FOUR Complete Office Forms
You must complete our intake document and policy and procedure packet at least one week before your scheduled appointment. These documents help us prepare for the assessment
FIVE Complete Evaluation
During the evaluation one of our therapists will work with your child to perform a variety of fun tasks. It is recommended that one parent/guardian be present but that you allow the therapist to take the lead and guide the session to ensure we capture an accurate picture of your child. You can expect to receive a report within two weeks of the last testing session.
